The dedicated staff at Finger Lakes Visiting Nurse Service provides clinically excellent and compassionate home health to individuals and families in the communities we serve. But what happens after discharge?

When a patient has been successfully discharged from home health nursing or rehabilitation services at FLVNS, they can still continue to receive private duty home care if they wish through our sister agency, Finger Lakes Home Care. In fact, with a physician’s order, anyone care receive care from Finger Lakes Home Care through their insurance carrier (if covered). Patients may also pay for the services themselves.

Finger Lakes Home Care is committed to client advocacy and the preservation of dignity, individuality, and privacy. The experienced and compassionate staff understand patient’s needs. The philosophy of Finger Lakes Home Care is to enhance the quality of life for its clients and their families by assisting them to remain safely at home. Services are available 24 hours per day.

Registered Nursing Services:

  • In-home assessment
  • PRI/Screen for nursing home or alternative placement

Home Health Aide Services:

  • Personal care assistance
  • Light housekeeping
  • Meal preparation
  • Medication reminders
  • Running errands, grocery shopping, and laundry services

Criteria For Admission:

  • Physician order for program services 
  • An assigned support person, whether family or friend, to help coordinate care
  • A safe home setting
