TeleHealthNew in 2010: TeleHealth is an in-home monitoring system which provides clinical information on patient’s health status. TeleHealth monitoring may be appropriate for individuals who suffer from heart or lung disease, diabetes, or high blood pressure. The daily monitoring system transmits patients’ heart rate, blood pressure, oxygen saturation levels, and weight to Finger Lakes Visiting Nurse Service via a telephone and allows day-to-day evaluation of patients’ stability. Changes in status are communicated with the nurse case manager and physician.
